We Work With Most Major Insurance Plans
Legacy Surgical Partners accepts most major commercial insurance plans, including Cigna, Blue Shield, Anthem Blue Cross Blue Shield PPO, United Healthcare, and often Aetna.
Coverage and eligibility vary by plan, and not all procedures are covered under every policy. Our staff will help you understand your benefits and guide you through the pre-authorization process.

Office Visits
Please bring your insurance ID card to every visit.
If your plan requires a co-payment, it is due at the time of service. Most annual exams are covered without a co-pay, so please inform our staff if your visit is for an annual exam.
We accept major credit and debit cards, personal checks, and cash.
Surgical Fees & Pre-Authorization
Before scheduling surgery, our team will verify your insurance coverage and, when applicable, begin the pre-authorization process.
This ensures you know what portion of the procedure is covered and any co-insurance or deductible amounts that may apply.
If your insurance covers gender-affirming or reconstructive surgeries, you will only need to pay your co-insurance up front.
We manage the pre-authorization process on your behalf and keep you informed at each step.
If you’re paying out of pocket, all fees for surgeon, operating room, and anesthesia are due in full three weeks prior to your scheduled procedure.
Medicare
Legacy Surgical Partners is not contracted directly with Medicare. Despite ongoing efforts, our office has been unable to obtain consistent coverage approvals through Medicare, and we therefore bill these procedures as self-pay.
However, our partner hospitals and anesthesia providers are contracted with Medicare and typically submit claims for their services directly.
This means you may still receive partial reimbursement for facility and anesthesia fees.
Please contact our team if you have Medicare and wish to review potential out-of-pocket costs or available options.
Medi-Cal / Medicaid
We are not contracted with Medi-Cal or Medicaid, except for San Francisco Health Plan.
Unfortunately, due to the extensive pre-authorization process required by these programs, we cannot currently accept other Medi-Cal managed plans or out-of-state Medicaid coverage.
We remain committed to advocating for broader access to gender-affirming care and are happy to provide letters of medical necessity or documentation to support patients seeking coverage adjustments through their employer or insurer.
Out-of-Network & Special Cases
For patients with plans that are out of network, many insurance carriers—including Aetna—will still authorize coverage on a case-by-case basis.
Our team will work with your insurer to obtain the necessary pre-authorization whenever possible.
If your plan lists “Transgender Surgery,” “Gender Affirming Surgery,” or “Genital Reassignment Surgery” as an exclusion, we encourage you to contact your employer or benefits administrator.
Our office can provide supporting documentation to request policy amendments or coverage exceptions.
